99mTc-sestamibi 신티그라피를 이용한 유방종괴의 감별 진단 (Differential Diagnosis of Breast Mass Using 99mTc-sestamibi Scintigraphy)

초록

Background 99mTc-sestamibi is a radiopharmaceutical for myocardial perfusion imaging and also known to be accumulated in lots of tumors. The purpose of this study is to evaluate 99mTc-sestamibi scintimammography as a potential tool in differentiating malignant from benign lesions of breast. Methods We acquired 99mTc-sestamibi scintimammograhy in 37 patients who had palpable nodule in breast. Seven hundred forty MBq (20 mCi) of 99mTc-sestamibi was injected intravenously and anterior image of chest on supine position, both lateral images of breast on prone position were obtained. The intensity of 99mTc-sestamibi uptake of breast mass was evaluated visually by comparison with cardiac activity. The results were compared with pathologic results or clinical follow up data. Results Nine patients were confirmed as breast cancer and all showed positive 99mTc-sestamibi uptake except one patient with carcinoma in situ (sensitivity 88.9%). There was one patient who had axillary lymph node metastasis but failed to show 99mTc-sestamibi uptake. There were three false positive uptakes in one fibroadenoma and two fibrocystic disease patients (specificity 89.3%). Conclusion 99mTc-sestamibi scintimammography is sensitive and specific tool to differentiate malignant from benign masses of breast.
