젠더규범과 청소년의 건강: 위험 성행동을 중심으로 (Gender norms and adolescent’s health: focusing on sexual risky behavior)

초록

이 연구는 청소년의 위험 성행동을 건강위험 행동의 하나로 보고, 청소년의 위험 성행동과 건강결과 간의 관계에서 젠더규범이 하는 역할을 규명하고자 하였다. 궁극적으로 다른 건강위험행동과 마찬가지로, 위험 성행동으로 인해 부정적 건강결과가 발생했을 경우, 낙인 없이 사회적 지원을 받을 수 있는 개입 지점을 정책 개선안으로 도출하고자 하였다. 이를 위해 Tolman의 통합적 생태학적 모형을 이론적 틀로 활용하였으며, 여성주의와 사회구성주의 관점에 따라 반구조화된 심층면담을 수행했다. 연구참여자는 20∼24세 후기청소년 10명(여성 7명, 남성 3명)이었다.
연구 결과, 한국사회의 성차별적 젠더규범은 다른 종류의 규제(나이주의, 가부장주의 등으로 인한 규제)들과 함께 규제적으로 작동하고 있음을 확인할 수 있었다. 그리고 성차별적 젠더규범은 제도화된 성교육을 통해 청소년들에게 내재화되고 재생산되고 있음을 확인할 수 있었으며, 체화된 규범과 가부장적 질서는 일상을 지배하고 있었다. 또한 연애 관계에서 발현되는 성적 이중규범은 성적 주도성을 상실하게 만들고 있었다. 성적 주도권의 상실은 위험 성행동을 ‘수행하기’보다 ‘당하게’하고 있었으며, 성역할 고정관념과 성적 이중규범은 성행동에 있어서 여성청소년의 동의 의사를 소거하고, 거절을 거부당하게 하는 역할을 하고 있음을 확인할 수 있었다.
따라서 청소년의 위험 성행동 수행으로 인한 건강위해를 예방하기 위해서는 유네스코에서 권고한 포괄적성교육으로 교육 방식과 내용의 전환이 필요하며, 콘돔의 가격인하를 포함한 자원 접근성을 용이하게 만들어야 하고, 청소년에게 안전한 보건의료서비스에 대한 접근을 보장할 수 있는 법적, 제도적 보완이 필요하다.

영어초록

This study is to understand the role of gender norms in the relationship between adolescents' risky sexual behaviors and their health outcomes taking adolescents' risky sexual behaviors as health risk behaviors. We intended to provide the policy proposal that would help the adolescents who were facing any health risks as a result of risky sexual behaviors receive necessary social supports without any social stigma.
We employed Tolman's integrative ecological model as a theoretical framework and performed a series of semi-structured interviews following feminism and social constructivism tradition. There were 10 participants, 7 females and 3 males, in their late adolescents of 20-24 years old.
Results showed that the sexist gender norms, with other controling measures such as ageism and patriarchy were playing a significant role in Korea. Our study found also that the sexist gender norms were internalized and reproduced through institutionalized sex education, and the embodied norms and patriarchal orders dominated their everyday lives. Furthermore, the sexual double standard present in their romantic relationship made them lose their sexual autonomy. It was found in our study that the loss of sexual autonomy had them not 'do' but 'forced to do' the risky sexual behaviors, and the sex role stereotypes and sexual double standards deleted female adolescents' agreement and denied their disagreement in their sexual behaviors.
We propose that, in order to prevent health risk outcomes due to adolescents' risky sexual behaviors, the sex education contents and delivery system need to make necessary changes following the UNESCO's recommendation of comprehensive sexuality education. Also, condom use needs to be more accessible including the cost. Further, legislative and institutional provisions to guarantee adolescents' access to safe public health services should be improved.
