암환자 배우자의 돌봄부담이 부부적응에 미치는 영향 -사회적 지지의 조절효과를 중심으로- (The moderating effects of social support on the relationship between care burden and marital adjustment in cancer patients' spouse.)

초록

본 연구는 암환자 부부의 결혼안정성의 중요성에 대한 인식을 바탕으로, 암환자의 배우자를 대상으로 암환자를 돌보면서 경험하는 돌봄부담과 부부적응의 관계에서의 사회적 지지의 조절효과를 검토하여 이들에 대한 사회복지 실천의 토대를 제공하는 것을 목적으로 수행되었다. 이를 위해 암환자 배우자 203명을 대상으로 설문조사를 실시하였고, 사회적 지지의 조절효과를 검증하기 위하여 위계적 회귀분석과 단순기울기 검증을 실시하였다. 연구 결과 암환자 배우자의 부부적응에 대해 돌봄부담의 부적 영향이 유의하였으며, 돌봄부담이 부부적응에 미치는 영향에 대한 사회적 지지의 조절효과가 유의한 것으로 나타났다. 이러한 결과를 바탕으로 암환자 배우자의 돌봄부담을 완화하기 위한 간병휴가제도 마련, 발병초기 보호자 교육 마련 및 심리정서적 지지 상담 등을 통한 개입과 고위험군에 속하는 암환자의 배우자 집단의 선별(screening)을 통한 집중적이고 장기적인 사회복지적 개입이 필요하다는 사회복지 실천에의 함의를 제시하였다.

영어초록

Stable marriage has significant effects on cancer patient's psycho-social adjustment, progress of the medical treatment, survival rate and the quality of life as well. Therefore, it is importance that social welfare related intervention and care are needed in order to further stabilize their marriage. However, there is a severe lack of empirical study about married cancer patient couples in Korea. Studies about how the spouse experiencing extreme distress while taking care of the cancer patient is almost non-existent. Purpose of this study is to lay the foundation for implementing social welfare practice for the cancer patient couples by focusing on the importance of stable marriage, on the effects that care burden has on the spouse of the cancer patient and the effects it has on their marital adjustment through evaluating these by moderated effect of social support. The survey was conducted with 203 spouses of cancer patients. Through this survey, it was revealed that the effect of the care burden and the social support on marital adjustment of the spouse of the cancer patient are found to be significant. About the care burden's effect on marital adjustment, the social support had found to be have moderating effect.
Based on these results, the conclusion that in order to lessen the stress related to care burden, system that provides vacation time for nursing the patient, providing the education for the care giver in the primary stage of the illness, counseling that provides psychological and emotional support and others are necessary. Moreover, in order to promote marital adjustment of cancer patient, the importance of strengthening the social support and not just the intervention of the social welfare for lessening the care burden had been provided by this research.
