초록

Purpose: This study sought to determine whether Korean hand therapy would decrease the incidence of postoperative nausea and vomiting (PONV) and discontinuation of intravenous patient-controlled analgesia (IV-PCA). Methods: In the experimental group, the capsicum plasters were attached to four points of Korean hand therapy (K-D2) from 30 minutes before the anesthesia to 8 hours after mastectomy. The primary outcome was the incidence of PONV in the first 24 hours after the end of anesthesia.
Secondary outcomes included discontinuation of PCA, rescue of antiemetic, additional analgesics, dosage of PCA, and degree of pain. Results: The analysis included 71 (Korean hand therapy group= 37, control group= 34) patients. There were no significant differences in the overall incidence of vomiting, the need for rescue antiemetic, and pain scores within the first 24 hours after mastectomy between the two groups, but the incidence of nausea within 2 to 24 hours after surgery (43.2% vs 88.2%, p＜.001) and the intensity of nausea during the first 24 hours after surgery (p＜.001) were significantly lower in the experimental group than in the control group. Compared to the control group, the experimental group had a lower incidence of PCA discontinuation (10.8% vs 61.8%, p＜.001), and a lower need for additional analgesics (p=.040) within the first 24 hours after surgery. Conclusion: This study demonstrated that Korean hand therapy can reduce the incidence and intensity of nausea and enable the continuous use of IV-PCA, thereby reducing the discomfort of patients who require additional painkillers after feeling pain.
